To become a client:

Step 1: Please call our office (480) 895-7133, to schedule a home visit from our professional case manager.

Step 2: During this home visit, our case manager will explain the services we offer, and work with you to select the services that are best for you.

Step 3: A short medical and socioeconomic history will be taken.

Step 4: A record will be established in our database, identifying you as a client. You may then request the services that have been selected for you.

Note: If your needs change, please call the office for a follow-up visit.
